Carol Hartman, RN

Outreach/Transition Coordinator


Carol visits health care facilities throughout the region to market and coordinate home health services and receive referrals to Sunshine Health Facilities. She speaks with nurses, doctors, and clients to establish the services they need and to obtain doctor orders for the services. Her primary role is working with case managers to determine what the client needs and then share that information to Sunshine’s clinicians and office support.

Her background as an RN gives her a practical understanding of the services nurses and doctors need to assist their patients best. Carol loves working at Sunshine. The organization’s small company atmosphere and Sunshine staff can give personal attention to each client and each employee’s needs. Carol enjoys being part of a collaborative team with the shared goal of providing excellent service and patient care.

Carol enjoys walking—which is a good thing as she is often busy attending her grandchildren’s activities and spending time with her own children and family.
